有相當比率的人工電子耳兒童,與聽常同儕相較下,有明顯的嗓音問題,他們的音高起伏較大,音量亦普遍較大,並有控制不佳而起伏不定的情形。由於聲帶功能運動具有增加喉部肌肉的協調性與控制性,和改善整體音質的功能,因此本研究透過聲帶功能運動並搭配波型圖的APP軟體提供視覺回饋,以期能改善人工電子耳兒童的嗓音品質,使其溝通更具效率。
本研究為10位配戴人工電子耳的國小學童進行六週的聲帶功能運動訓練,受試兒童每天需居家練習兩次,每次十五至二十分鐘,另外,研究人員每週會至個案家中進行一次介入,了解受試兒童的練習情形,並提供訓練與修正。
本研究採用Computerized Speech Laboratory與Multi-Dimens ional Voice Program進行聲學分析,以GRBAS量表進行聽知覺評估,並藉由嗓音障礙指數簡易版(VHI-10)評估受試兒童對於其嗓音障礙的自覺情形。根據前後測的評估結果發現,受試兒童在以下的分析指標皆有所改變:
(一)、聲學分析指標,包括頻率擾動係數、振幅擾動係數、頻率波峰變異、振幅波峰變異以及噪訊比等皆出現顯著性改善(p<0.05)。
(二)、氣動學分析的最長發聲時長亦呈現顯著延伸(p<0.05)。
(三)、嗓音障礙自覺評量:在介入後,個案自覺嗓音障礙對日常生活的影響有顯著減小(p<0.05)。
(四)、聽知覺分析:在拉緊聲方面,個案達極顯著改善(p<0.01),在整體嗓音品質、粗啞聲等方面皆出現顯著改善(p<0.05),但在氣息聲與嗓音無力等方面未達顯著差異。
研究結果發現,聲帶功能運動可有效改善國小學齡人工電子耳兒童的嗓音異常,因此建議將聲帶功能運動列入人工電子耳學童的聽能復健方案中,以改善其嗓音品質,並提高溝通效能。
